The purpose of nutritional education is to help you choose to take steps to improve the overall health, vitality and well-being of the body through nutritional education and the use of non-medical nutrition and non-medicinal modalities. The Nutritionist, Kourtney Simmang, does not diagnose or treat diseases, disorders or conditions.

The information and content is not intended to replace an individual relationship with a qualified health care professional and is not intended as medical advice. Kourtney encourages you to continue visiting with and being treated by your primary care physician or health care provider. Only a Licensed Medical Professional can diagnose certain ailments and prescribe treatments and medications for you. Do not ignore or delay seeking medical advice or treatment because of any information obtained from Kourtney. Consult your own Medical Professional regarding all nutritional supplements to evaluate if the supplements could adversely affect or react with any medications or pre-existing conditions. Stop any food or supplement immediately if you have any type of adverse reaction.

Kourtney is not not acting in the capacity of a doctor, licensed dietician, psychologist or any other licensed/registered professional. Kourtney will not diagnose, treat, or cure in any manner, any disease, condition, or other physical or mental human ailment.

In providing Nutrition Counselling Services to me, the Nutritionist, Kourtney Simmang, is relying upon the truth, accuracy and completeness of all information I have provided to her. No recommendations are intended to treat, diagnose, or prevent any condition. Any recommendations I follow for changes in diet, including the use of nutritional supplements, are entirely my responsibility and are considered self-treatment. Kourtney Simmang is in no way liable for my health or safety. In consideration of my participation in Kale Me Kourt Groups, I hereby accept all risk to my health, including injury or demise that may result from such participation and I hereby release the Nutritionist, Kourtney Simmang, on my behalf and on behalf of my personal representatives, estate, heirs, next of kin, and assigns from any and all costs, claims, causes of action and damages arising from any and all illness or injury to my person, including my demise, that may result from or occur as a result of my participation in the Nutrition Educational Services, whether caused by negligence or otherwise.

During your time with Kale Me Kourt Nutrition, you might be placed under the guidance of Sarah King (BS, NTP) and Courtney Coe (MS, RDN, LDN, CSCS) for wellness support. I release both Sarah and Courtney from any and all claims, and give them permission to support me in my health.

Due to the service nature of Kale Me Kourt, there are not refunds offered for the purchase of goods. I agree to not dispute any charges for group coaching or add-on's, and I understand that any disputes will be reported to the top credit bureaus, which can have an impact on my credit score. If I ever have a question about payment or billing, I will reach out the group support email (groupcoaching@kalemekourt.com) If I change credit card or debit card numbers, I will notify Kourtney 48 hours or more before scheduled payment is due. I agree to pay in full for the level of service that I select, and that upon engagement, I am committing to pay the full amount, even when selecting a payment plan.

I understand that any suggestions I undertake at Kale Me Kourt are undertaken of my own free will. I accept that the ultimate responsibility for my health care is my own and that Kale Me Kourt is here to support me in this. I release Kourtney Simmang from all legal claims. I understand that my nutritionist reserves the right to determine which cases fall outside their scope of practice, in which event an appropriate referral will be recommended. I hereby agree to assume full responsibility for any manner of loss, injury, claim or damage whatsoever, known or unknown, incurred as a result of same and I, my heirs, executors, administrators or assigns for any loss, injury, claim or damage sustained as a result of my attendance and/or participation. I have read the above release and waiver of liability, and fully understand its contents and voluntarily agree to the terms and conditions stated.

By working with Kourtney, you understand that she is a certified nutritionist consultant (CNC). She is not a dietitian, therapist, or licensed medical professional, and therefore you need to discuss and clear any and all changes to your lifestyle, food intake, exercise regimen, or medical treatment with your physician before implementing changes or habits suggested by her. You must discuss any and all changes to your diet, exercise regimen, supplements, medications, or lifestyle with your physician or qualified medical professional before implementing any suggested or offered changes, additions, or alterations to your lifestyle. Suggestions are for informational and educational purposes only.

I understand that my participation is necessary to derive the full benefit from Kale Me Kourt Groups, and that Kourtney is in no way responsible for offering additional 1:1 support or guidance for me beyond the package I have purchased. Included in Kale Me Kourt Groups are the following:

Live calls
12 question credits to be used for singular questions (with 3-5 business day response time)
Applicable handouts and resources at Kourtney's discretion
Call recordings
An HTMA test and GI map or DUTCH test
